From Admission to Discharge – Hospitalists Provide Complete Coordination of Care
Spooner Health is at the forefront of our commitment to care through our advanced practice nurse practitioner (APNP) hospitalist program. If you or a loved one is hospitalized at Spooner Health, one of our skilled APNP hospitalists will oversee your care from admission through diagnosis, treatment and discharge. Our hospitalists are available 24 hours a day, 7 days a week to answer questions, address any changes in your condition, coordinate treatment among multiple medical specialists and maintain contact with your regular physician.
Spooner Health’s advanced practice nurse practitioner (APNP) hospitalist program enhances care for hospitalized patients. If you or a loved one is hospitalized at Spooner Health, an APNP hospitalist will oversee your care from admission through diagnosis, treatment and discharge, updating and involving your physician as needed.
When patients are discharged from the hospital, they return to their primary care physician for follow-up and ongoing care, including any necessary specialty services.
Spooner Health’s team of hospitalists is dedicated to the care of patients during their hospital stay and to ensuring a smooth transition for patients after discharge.
